Form 1095-B
The school is required to provide each employee a Form 1095-B. This is a tax statement confirming your health insurance coverage status for each month during the year. Please contact a tax professional if you need help determining if this applies to your current tax situation. The school will furnish this form upon
request.
You may email hpatterson@mountdesales.net or you may come to the main office at 851 Orange Street, Macon, GA 31201 to pick up your form. These forms will be filed with the IRS, just as your W-2 is filed with the IRS each year.
