Free Sports Physicals by Piedmont Orthopaedic
All Mount de Sales student-athletes are required to have a physical, pre-participation form, and concussion form on file before they are allowed to participate in any team activities, including tryouts. Piedmont will be offering FREE physicals for all MDS student-athletes on Thursday, May 11 at 5:30 p.m. at Cavalier Fields.

The athletic department at Mount de Sales Academy is excited to provide you with a state-of-the-art digital pre-participation system, which will make your athletic registration process easier while keeping your student’s important medical information safe. With PlanetHS, your student’s health history information will carry over each year and all you have to do the following year is make any changes. This new process will save you time and paper, while giving you the convenience of filling out most forms via computer or mobile phone.
**It is very important that all parents fill out paperwork online prior to May 11.**
PlanetHS has made it easy for you to set-up your accounts and fill out the GHSA and school required information. The process is outlined in the following attachments and on the site, but the important first step is to create and link the parent and student accounts. Once accounts are set-up, the PlanetHS site will walk you through the process step by step with written and video tips as follows. We also are providing you with 3 documents, which will explain the process in more detail.
